Practical information to help your first appointment be as useful as possible.

What to Bring

Come prepared and the first visit goes further

A neurological evaluation depends heavily on history and prior investigation. The more of this you bring, the less likely it is that tests will need to be repeated.

A note on symptom diaries

For headache and migraine in particular, a record of the last four to six weeks is genuinely useful: how many days you had pain, how severe it was, what you took, and whether it worked.

A note on your phone is perfectly sufficient. It often reveals patterns that are difficult to recall accurately in a consultation.

For Referring Physicians

Referring a patient

Dr. Rajneesh accepts referrals for neurological evaluation and for chronic pain that has not responded to first-line management. Referrals are particularly welcome where the diagnosis is unclear, or where a neurological cause is suspected behind a chronic pain presentation.

How to refer

Call (832) 400-2780 or have your office send the referral together with relevant imaging reports and prior treatment notes.

What to include

A brief clinical summary, current medication list, relevant imaging or nerve study reports, and the specific question you would like addressed.

Communication back

A consultation letter is sent to the referring physician after the initial evaluation, and following any significant change in the plan.
